Can you help me coordinate information from multiple providers and specialists?

We can help you organize provider information, appointments, records, questions, and follow-up responsibilities. We do not clinically coordinate treatment or direct your providers’ decisions.

Yes. We can help you think ahead, identify questions and decisions that may arise, and create practical tools for tracking and preparation. Medical changes should always be discussed with your healthcare team.

Can SurePath help me create a practical plan for managing the non-clinical parts of ongoing care?

Yes. Your plan may address records, appointments, benefits, paperwork, bills, caregiving responsibilities, questions, deadlines, and anticipated transitions.

Can a caregiver or family member participate?

Yes, with the client’s permission. We can help clarify roles, organize shared information, and make it easier for everyone to understand what needs to happen next.
